Clinician scribes and patient companions can observe the same visit and produce different medications, follow-ups, symptoms, and instructions. Reconcilia does not merge them automatically. It aligns claims, cites available source material, labels agreement, omission, contradiction, uncertainty, and unsupported content, then routes differences to the clinician and patient under defined roles. A signed artifact proves what was reviewed and by whom, not clinical truth, diagnosis, completeness, or chart accuracy.
